Re: Chemical Inventory Reports
Federal and state laws require
facilities that use, produce, or handle hazardous chemicals to report
their hazardous chemical inventory to the local fire department
each year. Michigan law further requires each fire chief to
provide such information to fire fighters who may respond to an
emergency at your business. To assist the Kentwood Fire Department in
this effort, we ask that you complete the enclosed survey. If your
company does not have any hazardous chemicals, you still must
complete the form.
Indicate whether your site uses or
produces any of the chemical types listed. Check all the categories
that apply when a chemical has more than one characteristic; e.g., both
a Class 3 flammable and a Class 6 poison. Indicate the quantity
category for each chemical group on your site. You may need to
reference Material Safety Data Sheets (MSDS) or other reporting forms to
complete the survey. Your supplier, product labels, or chemical
dictionaries may be helpful in determining if any of the chemicals at
your facility fall into the specified categories. Complete each line.
Do not leave blanks. If you do not use a chemical group, mark the box
in the ?DO NOT HAVE? column.
If the chemicals on site meet or exceed
the specified quantities, we may seek additional information to help us
prepare site-specific response plans. You may be asked to provide
material safety data sheets, listing of the chemicals by name; maximum
quantities; and the locations of the chemicals at your facility.
If there is a change in use,
production, or quantity of hazardous chemicals at your firm, a revised
survey must be submitted to the fire department as soon as possible
after the change.
Please complete the survey and return
it within ten (10) days to the Kentwood Fire Department by mail to P O
Box 8848, Kentwood, MI 49518-8848, fax to 616-554-0799, or by email to
dumbauldL@ci.kentwood.mi.us. You can access the survey form via
the City of Kentwood?s website at
http://www.ci.kentwood.mi.us/fire/documents.asp under the
Documents tab/Chemical Survey Inventory 2010. All responses will be
kept on file in the fire department.
